New Patient Registration

PreRegistrationIf you wish to register at Bishop's Place Surgery you can call in and request a registration form.  If you want to complete the form whilst you are here please bring with you an item of photograph identification, ie passport or driving license.  If you have neither of these you could use a utility bill detailing your address. 

Alternatively you could click on the link below to access the registration form.  When you have completed all of the details please bring it in to the surgery together with your identification, as above.

When you register you will be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.  Sometimes you will be invited to attend for a new patient medical with a member of our Practice Nurse Team.
